CMS Accepting Applications for New Payment Model for Serious Illness Care

The Centers for Medicare & Medicaid Services (CMS) has released a Request for Applications (RFA) for practices in 26 regions to participate in a Seriously Ill Population (SIP) option under its new Primary Care First (PCF) payment model demonstration.
