The 2022 Ascend Leadership Forum is a two-day in-person intensive to support hospice and palliative care professionals as they develop the capabilities required to lead their organization in implementing new directions. This program includes a strong emphasis on interdisciplinary faculty and learners from diverse backgrounds and practice settings.
Even our most gifted leaders are weary and have had their leadership skills tested these last few years. If you need a safe place to reflect and recharge by being around other hospice and palliative care leaders, the 2022 Ascend program is for you.

Transportation
Attendees can fly into either O'Hare International Airport (ORD) or Chicago Midway International Airport (MDW).
O'Hare is 16 miles (30 mins) from the Hyatt Lodge, Chicago Midway is 20 miles (40 mins) from the Hyatt Lodge. Cost is approximately $35-$50 one-way via taxi, Uber/Lyft.
Parking Information
Self-Parking is complimentary. For valet, it is $10/day and $18 overnight. AAHPM does not validate for parking. Rates are subject to change. Please contact the hotel for the most up-to-date parking rates.
Accredited Continuing Education
The American Academy of Hospice and Palliative Medicine (AAHPM) is accredited by the Accreditation Council for Continuing Education (ACCME) to sponsor continuing medical education for physicians.
The American Academy of Hospice and Palliative Medicine designates this live activity for a maximum of 15.75 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
Successful completion includes attesting to meaningful activity participation and completion/submission of activity evaluation within the time frame designated.
Maintenance of Certification (MOC)
This activity is planned in the context of one or more of the ABMS/ACGME Competencies including Interpersonal and Communication Skills, Professionalism, Systems-Based Practice and Work in Interdisciplinary Teams, AAHPM is seeking approval for Maintenance of Certification points for the 2022 Ascend program.
It is the CME activity provider's responsibility to submit learner completion information to ACCME for the purpose of granting respective Board MOC program credit or points. By submitting your evaluation, you grant AAHPM permission to submit complete information to ACCME for the purpose of granting respective BOARD MOC program credit or points as applicable.

Cancellation Policy
All cancellations must be made in writing. A $150 processing fee will apply to all cancellations. No refunds will be made on cancellations postmarked after October 1, 2022. All refunds will be processed after the course concludes.
If the course moves to a fully virtual meeting, registrants of the in-person meeting will automatically be transferred to the virtual meeting.
AAHPM reserves the right to substitute faculty, cancel or reschedule sessions unforeseen circumstances. No refunds can be made for lodging, airfare, or any other expenses related to attending the summer course offerings.
